Do I need a prescription or referral from my doctor?
Generally, no. Wisconsin is a Direct Access state for physical therapy, which means patients are allowed to seek evaluation/treatment for musculoskeletal injuries from a licensed physical therapy without a referral from a physician. However, there are some exceptions and PT will advise based on your needs.

Will my PT communicate with my doctor?
Yes. EVOLVE will communicate with any of your medical providers to provide the best patient care possible.

How long are appointments?
The initial PT evaluation appointment is 60 minutes. Follow-up treatment sessions may range from 45-60 minutes depending on the recommended treatment plan set at your first visit.

What will my first visit be like?
The first and most important step will be to listen to what brings you here. Then a comprehensive evaluation of your medical history, posture, mechanics, and movement will be conducted. A discussion about your goals and lifestyle will then drive your individual treatment plan. A collaborative approach of education and answering questions regarding your care is of the utmost importance in empowering you towards your recovery. You will be in a private treatment space to ensure your comfort. The initial appointment is 60 minutes.
What should I bring to physical therapy?
For your first visit, please bring any relevant medical documents (MRI or X-ray results, medical records, etc.), a prescription (if you have one), and the completed required intake forms on our website.

Do you take insurance?
Currently, EVOLVE is a cash-based practice ONLY and no longer accepts insurance.

In a cash-based treatment model, the physical therapist provides services on a flat-fee basis. Payment is at time of service. Patients then receive the following benefits:
